Benign Prostatic Hyperplasia

Introduction The process of urination is vital to the body for the removal of certain waste products. Normally, when the bladder gets full, urine is emptied from the body through the urethra in a process called urination. In urinary retention, a person’s bladder fills up and there is an urge to urinate but he is unable to completely empty his bladder. With chronic urinary retention, a person may be able to urinate, but he has some trouble starting a stream or emptying your bladder completely. He may urinate frequently, may feel an urgent need to urinate but have little success when he gets to the toilet; or he may feel he still have to go after having finished urinating. With acute urinary retention, he can't urinate at all even though he has a full bladder. Acute urinary retention is a medical emergency requiring prompt action. Acute urinary retention (AUR) is an extremely uncomfortable and potentially life-threatening condition characterized by a sudden inability to urinate associated with intense suprapubic discomfort. It is most often secondary to obstruction, but may also be related to trauma, medication, neurologic disease, infection, and occasionally psychological issues. Acute urinary retention (AUR) is one of the most significant, uncomfortable and inconvenient event in the natural history of benign prostatic hyperplasia (BPH). BPH is the virtual universal overgrowth of the prostate gland in men as they age. Various factors affecting BPH are age, genetic makeup, testosterone levels, and environment. As the prostate enlarges it compresses the urethra causing obstruction to urine flow. The urinary stream becomes smaller; there may be difficulty in initiating the stream, dribbling, and intermittent flow, frequency of urination, getting up at night to void (nocturia), inability to empty, or even inability to void at all (retention). One week prior to consultation he had difficulty in urinating and presence of blood in the urine b. Medical History The patient claimed that he has hypertension and diabetes mellitus type II. Anatomy and Physiology Prostate gland The prostate is a walnut-sized gland that forms part of the male reproductive system. The gland is made of two lobes, or regions, enclosed by an outer layer of tissue. As the diagrams show, the prostate is located in front of the rectum and just below the bladder, where urine is stored. The prostate also surrounds the urethra, the canal through which urine passes out of the body. Scientists do not know all the prostate's functions. One of its main roles, though, is to squeeze fluid into the urethra as sperm move through during sexual climax. This fluid, which helps make up semen, energizes the sperm and makes the vaginal canal less acidic. Urinary Bladder The urinary bladder is a muscular sac for storing urine. The triangular base of the bladder, the trigone, is defined by the two ureters that deliver the urine and the one urethra that drains the urine. When empty, the bladder collapses, and folds (called rugae) from in the bladder wall. As it fills, the folds become distended and the bladder  becomes spherical. The wall of the bladder consists of three layers similar to those of  the urethra: the mucosa, the muscularis (here called the detrusor muscle), and the adventitia. Circular smooth muscle fibers around the urethra form the internal urethral sphincter. As a man matures, the prostate goes through two main periods of growth. The first occurs early in puberty, when the prostate doubles in size. At around age 25, the gland begins to grow again. This second growth phase often results, years later, in BPH. Though the prostate continues to grow during most of a man's life, the enlargement doesn't usually cause problems until late in life. BPH rarely causes symptoms before age 40, but more than half of men in their sixties and as many as 90 percent in their  seventies and eighties have some symptoms of BPH. As the prostate enlarges, the layer of tissue surrounding it stops it from expanding, causing the gland to press against the urethra like a clamp on a garden hose. The bladder wall becomes thicker and irritable. The bladder begins to contract even when it contains small amounts of urine, causing more frequent urination. Eventually, the bladder weakens and loses the ability to empty itself. Urine remains in the bladder. The narrowing of the urethra and partial emptying of the bladder cause many of the problems associated with BPH. Pathophysiology Urinary retention can be caused by an obstruction in the urinary tract or by nerve problems that interfere with signals between the brain and the bladder. If the nerves aren't working properly, the brain may not get the message that the bladder is full. Even if you know that your bladder is full, the bladder muscle that squeezes urine out may not get the signal that it is time to push, or the sphincter muscles may not get the signal that it is time to relax. A weak bladder muscle can also cause retention. As a man ages, his prostate gland may enlarge. Doctors call the condition benign prostatic hyperplasia (BPH), or benign prostatic hypertrophy. As the prostate enlarges, the layer of tissue surrounding it stops it from expanding, causing the gland to press against the urethra like a clamp on a garden hose. As a result, the bladder wall becomes thicker and irritable. The bladder begins to contract even when it contains small amounts of urine, causing more frequent urination. The pathophysiology of bladder outlet obstruction in men with BPH has been attributed to both static and dynamic factors. The static obstruction is due to the bulk enlargement of the prostate encroaching upon the prostatic urethra and bladder outlet, whereas the dynamic obstruction is related to the tension of prostate smooth muscle. When urine stays in the bladder, however, bacteria have a chance to grow and infect the urinary tract. If urine backs up into the kidneys, permanent kidney damage can lead to reduced kidney function and chronic kidney disease. Physical mental illness adolescents Essay

Adolescence is a period during which many learning tasks must be accomplished. Even adolescents whose development is normal may have problems. Some adolescents, however, have major problems in achieving a satisfactory adjustment to the demands of home, school, and community. They may engage in unacceptable behavior; or their physical, social, emotional, and mental development may be slower than that of other adolescents. This research paper presents some representative illnesses of adolescence; the illnesses covered include problems of physical and mental illnesses. The physical illnesses Theoretical Framework Adolescence is unknown in many nonindustrialized countries. Instead, adulthood begins with the onset of puberty and is commonly celebrated with traditional rites of passage. With the advent of universal free education and child labor laws in Western countries, children, who otherwise would have entered the adult work world by the time they reached puberty, entered a period of life during which they developed an adult body yet maintained a childlike dependence on parents. Formal study of this transitional period between childhood and adulthood, known as adolescence, began with the work of G. Stanley Hall at the beginning of this century (Proefrock, 1999). But adolescence became a major field of study only in the past few decades. In fact, the Annual Review of Psychology did not include a review of research on adolescence until 1988- its 39th volume (Petersen, 1998). Adolescence has been celebrated in myth and fiction as a time of joy, with few pressures and demands. Is adolescence a carefree and happy stage? While many adolescent probably remember good times, they also remember times of unhappiness and stress, too. Moreover, most adolescents recall fears and doubts and periods of insecurity, along with unpleasant feelings about the kind of person they were. If so, they’re recollections are not unusual. Contrary to the romanticized version of adolescence as a carefree time, many illnesses may arise in this period. A truly carefree adolescence is rare, if not impossible. In spite of the illnesses that adolescents face, and the troubled behaviors that they may manifest, most adolescents do not become identified as â€Å"problem children. † This research paper will examine some illnesses in adolescent stage considered to be manifestations of disordered functioning. Causal factors will be presented for each illness, and treatments will also be discussed. Empirical Evidence In considering the problem of illnesses in adolescents, an important variable must be remembered: Adolescence is a very short period. Ideally, they develop from â€Å"primitive organism† to mature, stable adults. During the course of development, some behaviors may frequently be bothersome to others, or may cause psychological pain for the adolescent. Fortunately, these behaviors often disappear with time; the child â€Å"grows out† of them. Many theorists view life as a series of developmental periods through which children progress. During this progression, many tasks must be completed if the child is to become a reasonably well-functioning human being. How well each of us resolves these development tasks depends on many factors, including our genetic endowment, physical environment, and psychological support we receive from those raising us. Rather than review the development stages presented by one theorist, we will look more generally at what faces the child. Defining Illnesses Many criteria have been suggested for distinguishing whether the behavior of a child is to be considered an illness. A child’s behavior may not meet all these criteria, or even most of them, and still be of concern to a parent. For example, a teenager who is fearful about school might not be diagnosed as suffering from disorder, but the child would benefit from parental understanding and assistance in resolving this fear. The application of a formal diagnosis to many adolescent illnesses may result in negative labeling effects for many diagnosed adolescent, who are likely to carry the stigma of diagnosis throughout their school career. The more severe labels, may correctly or incorrectly influence teachers` perceptions of the asserts and deficits of adolescents. If an adolescent is expected to be a problem student because of a diagnostic label the teacher may well behave in a way that elicits problem behavior from the adolescent. The label may lead to a self-fulfilling prophecy. Mental Illness The term mentally ill is frightening to many people. Movies, books, and magazines often depict mental illness in frightening ways. In some cases, adolescents suffering from a mental illness do act unpredictably or even dangerously. With proper diagnosis and treatment, most of the symptoms of mental illnesses can be controlled. It is tempting to distinguish healthy adolescents from adolescents with mental illness problems. However, there is often a fine line between mental health and mental illness. It is important to understand that mental illnesses vary in their severity. For example, many adolescents suffered from various levels of anxiety or depression. Others have suffered from serious mental disorders with biological origins. Education about the adolescents` mental illness is vital for those with mental health problems as well as for the adolescents` friends and family. Obsessive-Compulsive Disorders. Approximately 5 million adolescents suffer from obsessive-compulsive disorders. An obsessive-compulsive disorder is an illness in which people have obsessive thoughts or perform habitual behaviors that they cannot control. People with obsessions often have recurring ideas or thoughts that they cannot control. People with compulsions feel forced to engage in a receptive behavior, almost as if the behavior controls them. Continual handwashing, counting to a certain number while using the toilet, and checking and rechecking all the light switches in the house before leaving or going to bed are examples of compulsive behaviors. Some compulsive behaviors that are more harmful include pulling out one’s hair and other forms of self-mutilation. The causes of obsessive-compulsive disorder are difficult to isolate. Some theorists believe that sufferers engage in compulsive behaviors to distract themselves from more pressing problems. Until recently, behavioral therapy, which focuses on controlling and changing behaviors, has been the common treatment for sufferers of obsessive-compulsive disorders. However, research now indicates that some of these disorders may be caused by a lack of the neurotransmitters serotonin in the limbic system. In the early 1990, a drug called clomipramine (Anafiranil) was released for prescription use. Researchers believe that chlomipramine alters the way serotonin is used in the brain. When used in conjunction with behavioral therapy, this drug has been found to be helpful in alleviating symptoms of obsessive-compulsive disorder. Imagine a warm summer day, the wind lightly blowing across bare skin with the warmth of the brilliant sun causing you to get minor goose bumps. In the background you smell of manure and dirt. You go inside and grab a cool and refreshing glass of homemade lemonade and go back outside and sip on it; savoring the sour taste of fresh lemon but at the same time the sweet relief of the sugar. As you stand outside you can’t help but notice all the agriculture that is going on around you from the diesel powered CASE IH 2500 Combine to the dairy farm just down the corner. Agriculture is all around you. In a society growing as rapidly as we are today it astonishes me how many people don’t know how all the food that we eat is produced. If you think about it, all the food we eat starts out with a simple seed, that seed is then planted and who are the people that harvest and tend to those plants, Farmers. Agriculture just doesn’t stop at farming, there is so much more. The stereotype for agriculture is always plants and that’s not it. Agriculture is so broad and so old, many things make it up. Agribusiness; an industry engaged in the producing operations of a farm, the manufacture and distribution of farm equipment and supplies, and the processing, storage, and distribution of farm commodities; this broad major separates into many other different majors such as Agricultural Law; Agricultural Economics and many more. These certain majors would not exist if it wasn’t for the first person long before there was written history who picked up a seed and decided to plant it in the ground and watched what happened. Besides studies there are also many different organizations that involve agriculture such as FFA (Future Farmers of America) and 4-H. BUniversity and Hornbrook is a research assistant. I feel very comfortable using this article. It gives me the base knowledge I need for my paper. In my opinion, forced ranking does not serve to improve results for any organization and does not motivate employees. To start with, whenever forced ranking results are completed, the majority of the employees fall at the mediocre level of the curve. Therefore, they feel comfortable that their jobs are still secure. This would lead to constant workers input that only saves them from being ranked as the poorest in an organization. In addition, the forced ranking does not portray the real picture of quality in any employee’s effort. When we consider motivation, forced ranking may sabotage the performance of highly rated employees. This is because, in any given appraisal activity, the method produces a normal curve irrespective of the performances staged by the employees (Pinder, 2008). In this case, an employee may have performed to the expected levels and capacity. These employees would expect to be recognized as having worked within their capacity thus providing exceptionally good results. However, they would still be ranked as a poor performer simply because some other employees have slightly better performances. According to the expectancy theory, the anticipation of the employee would affect his or her motivation towards work if the forced ranking places this kind of an employee at the poor performers’ level. The result would be a discouraged employee whose expectations of a reward do not come to be (Schultz & Nembhard, 2006). The obvious reaction after such an appraisal would lead to the employees comparing their respective performances. The poorly ranked employees would feel that the appraisal results were unfair whenever the perception of equal performances is imminent. According to the equity theory, the negative perception to unfairness would affect the employee’s complianc e to perform to their capacity. Furthermore, some employees who are ranked higher would compare their rewards to others of similar positions (Spector, 2008). Analysis The ‘family and mother I love you’ ad portrays the roles of parents in the family: the father as the ‘pillar’ and ‘shelter’ and mother as the light and guardian. It is a metaphorical ad in the sense that it only uses symbols to communicate its message. The word ‘family’ is at the center of the ad and it shows the changes the ‘family’ went through. Our parents took care of us when we were young and still helpless, and so it is our responsibility to take care of them when they get old and weak. This form of familial responsibility is embraced in most Asian cultures. In many Asian countries, the children are expected to take care of their parents when they grow up. It is a disgrace to leave the parents in an institution or transfer the responsibility of caring for them to a relative or professional caregiver (Adamson 88). On the other hand, in several Western countries, parents expect their children to be independent and learn to take care of themselves, not their parents (Rama 24). However, in more traditional Western homes, the family is still the most important part of children’s lives. There are also cultural stereotypes used. The parents are shown to be the loving and self-sacrificing people in the family. It suggests that parents should never be disappointed by their children. It tries to say that we are here because of our parents and that fact alone is enough to make us remember our responsibility to our parents. The Creation Story of Ancient Babylon Essay The Enuma Elish is one of the most famous stories in the Babylonian corpus of mythology. It is a creation story that is erroneously through to have led to the creation story in Genesis. Some, such as Bracher (2006) hold that this story is no more than the recitation of much older stories personifying the agricultural movement between birth (Spring) and death (later fall, after the harvest). The two major figures are Tiamat and Apsu, both gods of water, but two distinct properties of water: the one, the water used to fertilize fields, water that is fresh and good, but the other, salty and chaotic, the goddess of the Ocean, that is Tiamat. Timat, the female, with her husband Apsu gives birth to two gods of intermediate rank: that of the sky, and their names are Anshar and Kishar. These gods begin, like so many gods do, by behaving badly, and they ultimately need to be destroyed. Eah, the grandson of Tiamat, once this plan to have them destroyed is realized, he seeks to murder his grandfather Apsu. Once this is done, Eah and Damkina, his wife, give birth to the chief of the Babylonian pantheon, Marduk, the patron of the city and of Babylonian civilization. So far, it is quite a stretch to see how this has anything to do with the story of creation found in Genesis. Marduk is similar to the Greek god Apollo, for he the god of the sun, of geometry and order, all things necessary to build a city. He is the god of â€Å"re-creation† or re-creating an allegedly bad earth through the ingenuity of man, urban, civilized man. Once the murder of Apsu is discovered, Tiamat marries again and seeks to wreak havoc on the children and grandchildren. In the first tablet (section 90), Marduk is described in this way: He rendered him perfect and endowed him with a double godhead. Greatly exalted was he above them, exceeding throughout. Perfect were his members beyond comprehension, Unsuited for understanding, difficult to perceive. Four were his eyes, four were his ears; When he moved his lips, fire blazed forth. Tiamat, as said before, is chaos. She is the sea, not only dangerous, but useless in that one cannot drink it and one cannot grow crops by watering them with it. She, however, is a perennial element in the universe, the chaos of which all urban, civilized societies fear. The second tablet says: â€Å"Her decrees are firm, they are beyond resisting. † In other words, all civilizations fail. Man and technics can only go so far in taming her. He is the primal nature seeking revenge against the forces of man who have slashed into her with civilization: with technology (that Babylon had in abundance,) building of stone and cutting down trees of fuel and building materials. It seems here that Marduk is this god of civilization, that needs to â€Å"cut into† Tiamat in order to build their civilization. That the Babylonians had learned to harness water might have something to do with this fear of chaos. At the end of section 20 of Tablet II, this description of Tiamat’s army is given: She has set up the Viper, the Dragon, and the Sphinx, The Great-Lion, the Mad-Dog, and the Scorpion-Man, Mighty lion-demons, the Dragon-Fly, the Centaur Bearing weapons that spare not, fearless in battle Many of these objects and creatures became part of later mythological constructions. The Sphinx is the most fascinating, but its adoption by Egypt makes little sense, not only in that the two civilizations were bitter enemies, but that if the Sphinx is part of the arsenal of disorder, in that it works for Tiamat, how was it adopted as a major figure by Egypt? The Great Lion was used in Israel and Ethiopia, the Dragon is well known throughout ancient mythology of both eastern and western background (a dragon is just an old name for the dinosaur). It seems that this anti-pantheon is very important to mythology and might speak of either the land based forces of chaos and their affect on man (i. e. the scorpion man). In the meantime, Eah has learned of Tiamat’s plan, but it is clear that resisting her is futile. As both Eah and Anu both fail to stop this vengeful symbol of chaos and the passions, there is great fear from the progeny of the first couple, through it must be said that this second tablet is damaged, and only so much can be taken from it. The only one that has the ability to stand up to the forces of chaos now is Marduk himself, and ths is the purpose of tablet III. Significantly, section 130 of Tablet III has Marduk confronting chaos with rhetoric of civilization: Unalterable shall be what I may bring into being; Neither recalled nor changed shall be the command of my lips! Now hasten here and promptly fix for him your decrees, That he may go forth to face your mighty foe! When Lahmu and Lahamu heard this, they cried out aloud, All the Igigi wailed in distress: How strange that they should have made this decision! We cannot fathom the doings of Tiamat! They made ready to leave on their journey, All the great gods who decree the fates Several issues bear discussion at this point. First, Marduk, the god of order and civilization, says that his life will be forever. â€Å"Unalterable shall be what I bring into being. † But what he brings into being is identical to Apollo. Civilization and order, man’s fight against an alleged â€Å"mal-formed† universe, the very heart of Gnosticism and the secret societies who claim ( to this day) to have preserved the wisdom of Apollo, Marduk and Osiris. A council of the gods is called in the beginning of Tablet IV. This council suggests a sort of divine oligarchy of power that will be imitated in Babylon, an oligarchy of wealth, as all civilizations are. The wisdom of Marduk, it seems, is only for the powerful. The gods need to test his abilities, and a test is the destruction and re-creation of matter, things that science, with some reservations, says is impossible. The creation and re-creation of prime matter (really any matter) is the real holy grail for science, since it would permit the scientific elite to create (or re-create) the world nearly from zero, it is the grail of true revolution (cf Hoffman, 2001, for a detailed discussion of this). He passes this particular test and makes a cloak disappear and re-appear, a trick that has substantial ritual overtones. What is interesting here is that Marduk is no dictator, he is commissioned, as some say, to fight Tiamat by the council or pantheon of gods, all springing from the original union of Tiamat and Apsu. His weapons are the four winds of destruction, which seems to suggest that the forces of order can use, but not necessarily control, the forces of disorder for their own purposes (e. g. the water wheel, or the use of wind to harness power). It is this wind, and the chariot of destruction that Marduk rides, that eventually with destroy Tiamat: she is â€Å"inflated† by the use of the winds of destruction, and she is then killed by an arrow. With the carcass of Tiamat, Marduk will fashion the earth, with its laws, joining form with matter, as well as â€Å"setting the bounds† of the waters in an ordered universe. I’m still waiting to see exactly how this has anything to do with the Genesis story that many authors claim show â€Å"unmistakable† connections (see Heidel, 1966). At the very end of Tablet IV, at section 150 (which is apparently cut off) it is written: He crossed the heavens and surveyed the regions. He squared Apsus quarter, the abode of Nudimmud, As the lord measured the dimensions of Apsu. The Great Abode, its likeness, he fixed as Esharra, The Great Abode, Esharra, which he made as the firmament. Anu, Enlil, and Ea he made occupy their places. This is of immense importance, since out of chaos, civilization is born, everything, both man and matter, â€Å"has its place† and function with the rationally organized whole. But in order to do this, Marduk and his allies needed the â€Å"tablets† of knowledge that the goddess of Chaos had. But Tablet IV says that â€Å"these are not rightly his† (first line, section 130). Hence, the manipulation and destruction of the pristine nature (rather than chaos) is not the right of civilization. Marduk has created civilization and ordered the universe with form and number, law and regularity that civilization will take as its hallmark. But Marduk has no right to do this. His is the right of conquest. Tablet V is the most interesting of all the sections of this story. It is really the description of the infusion of order and number into chaos. It is the creation of the universe by the infusing of wisdom, that is, form, into chaos, that is, matter. The seasons of the agricultural year are ordered in the cycle of birth and death that already plants the seeds (so to speak) of all civilization’s eventual destruction. He creates the planets and their movements, and lastly, given the law giving of the natural world, he creates the human city, the lowest of the orders of nature, law of the city is the law of form in matter. It is the later natural law theory of human relations. This section of the tablet sums it up: Below I have hardened the ground for a building site, I will build a house, it will be my luxurious abode. I will found therein its temple, I will appoint its inner rooms, I will establish my sovereignty. When you come up from the Apsu for assembly, You will spend the night in it, it is there to receive all of you. When you descend from heaven for assembly, You will spend the night in it, it is there to receive all of you. I will call its name Babylon which means the houses of the great gods, I shall build it with the skill of craftsmen (section 130) This is likely the most important section of the story, in that it describes, not an Adam and Eve in a non-civilized paradise ordered by God, but civilization, the domination of men over nature under the decrees of Marduk. Nothing more different could be imagined. It is worth noting that only after Cain had killed his brother is the first city formed. The city is the place of sin, the place where the elite oppress the poor in the name of law and order. In the Babylonian story, it is the opposite, the very structure of Marduk’s wisdom is manifest in the city, that is a city only in that it is established by the sovereignty of Marduk. Tablet VI is the story of human creation, again without the slightest relation to the Hebrew story. Man, in short, is created by the blood of the second husband of Tiamat, the leader and instigator of the revenge on Tiamat’s progeny, and that is Kingu. Man is created with the blood of the first rebel, the consort of the goddess of chaos. Man is created, basically, to work as slaves for the gods, to manifest their wisdom and order at the lowest, material level. Several issues should be taken from the above: first, that man is a creation of two forces, light and darkness, chaos and order, wisdom and passion. These are opposite in the sense of bearing dialectical movement to higher and higher levels of synthesis. Second, that this is the pagan wisdom so beloved of the Masons and other societies, it is the beginnings of magic. Magic, given the above, is the ability to balance the order of the universe with its underlying drive to destruction and atomization. Force is at the root of this synthesis: force brought order and chaos together, form and matter. Force destroyed Tiamat, force created the world. Hence, all is force, all is manipulation. Magic takes its start from here. Man will not rest content begin the plaything of the Gods. He is made from the blood of rebellion, yet the order of the universe is to be found in him. This IS the very foundation of magic: making of one’s own order, not the god’s order. The creation and re-creation of matter in the council where Marduk proves his mettle is a symbolic and ritual form that shows the coming rebellion of men: to seek the power of the gods for themselves, for them to become gods themselves. This later myth is brought to fruition in Prometheus and Lucifer. Marduk and his creation of man is the fulness of man’s development, through it is far from clear that this development is a good thing. Civilization partakes of both forms of existence, chaos and order: it uses order in its general affairs, seeks the domination of Tiamat through technology, but also uses chaos, things such as subterfuge, massacre in war and other devices to maintain this order. Order is self defeating in that it must, like Marduk himself, use disorder in the maintenance of order. These two principles of the universe are at the root of all paganism: the moral equivalence of light and dark, order and disorder. There is no sin, only inappropriateness, something that any good magician would know. The nature of pagan religion here is manifest in the last Tablet: Who produces riches and treasures, establishes abundance; Who has turned all our wants to plenty; Whose favoring breeze we felt in sore distress. Let them speak, let them exalt, let them sing his praises! (Section 30) Marduk is not a god of mercy or of goodness, but he is the later Baal, the god who one approaches to ask for things. Through civilization, Marduk gives prosperity and treasure, but at a price. Civilization demands sacrifice, sacrifice through war, accident, disease and overcrowding. It seems that only the elite will share in Marduk’s wisdom. It is the continued multiplication of want and desire. Challenges to Construction Project Management The complexity of clients demands, together with the increasing complexity of the construction industry, particularly as a result of technological developments, has over the years resulted in specialisation within the industry. Variety of professions have developed as separate skills (e.g. Architecture, Quantity Surveying, ME Engineering etc.) and even on a small project all of those skills are involved. The key to the management of construction projects is therefore the way in which the contributors are organised so that their skills are used in the right manner and at the right time for the maximum benefit to the client. There is little point in the construction industry developing its skills if they are not then implemented effectively.(Walker, 2002) Scenario issues In addition to the professions already mentioned, every construction project involves many of the following additional stakeholders: Multiple regulatory bodies, local residents, statutory undertakers, end users etc. Internal and external stakeholders would be involved in different stages of the project. Early and comprehensive stakeholder identification and analysis, together with regular monitoring and updating, is necessary in order to deal successfully with all stakeholders and defuse many possible project obstacles. (Guerin, 2012) Successful Construction Project Management A project may be considered as successful if the building is delivered at the right time, at the appropriate price and quality standards, and delivers the client with a high level of satisfaction. Effective project management seeks to make sure that these objectives are achieved, but nonetheless, completing large construction projects on time and on budget depends on a high degree of synchronization among many stakeholders which requires the entire team to work in collaboration. The need for collaboration arise and are depended on level of uncertainty, interdependence and complexity of the project. Issues with construction industry and its link with collaboration was recognized in the Latham Report (Constructing the Team) in 1994, and again reinforced by the Egan Report in 1998 (Rethinking Construction)(Gardiner, 2014) .It is fair to say, that collaboration is even more needed in todays industry with its complex chain of supply and introduction of specialist subcontractors and supplier design. In fact, some types of punishments can actually cause the child to become even more rebellious. The child can experience a sense of worthlessness after being punished again and again. Children do not act out because they are â€Å"bad.† They act out in the hopes of receiving some kind of response or reward. In the past, children who acted out were classified as â€Å"bad kids† therefore were isolated or punished and often wound up slipping through the cracks of the educational system. Recently, some teachers have been attempting to help the behaviorally challenged child. Instead of isolating, punishing, labeling or ignoring a child, with work we can help them to become a part of the class. Teachers also have the power to help the child who would have once been considered a â€Å"lost cause† to learn. Many researchers have worked on learning about the causes of behavioral problems and possibly more importantly, have suggested some solutions to the problem. Behavioral theorists include B.F. Skinner, E. Thorndike, and William Glasser to name a few. Although their research and theories go by different names they all have one thing in common. All o f the above theorists are, in effect, saying that we are not going to change the child’s behavior by changing the child. We must change our reaction to the behavior in order to change it. The idea that bad behavior should not be rewarded is a basic concept. When one rewards any kind of behavior, bad or good, that behavior will continue. Children are especially quick to catch on to what kind of behavior will get them the result that they desire. B.F. Skinner, a behavioral theorist, says that â€Å"when a particular Stimulus–Response (S-R) pattern is reinforced (rewarded), the individual is conditioned to respond.† Skinner’s theory is known as Operant Conditioning (Skinner, 1950-71). Although it seems basic or common sense it is easy to forget that each behavior displayed addresses a need. That need may be attention... ...t every student will understand or grasp every lesson. The important thing to remember is that the student who tries needs to be rewarded too. When using the reward system, teachers need to make the result that we desire the same result that the child desires. This is very tricky. The solitude she finds herself in is seen in poem XVIII: â€Å"Close between grief and anger, a space opens where I am Adrienne alone. And growing colderà ¢â‚¬  (13-14). This is the exact point where we find out her sadness and loss she has experienced. I will focus on this side of the spectrum, Adrienne Rich’s transition into solitude. Poem XVIII starts off with rain in the city, and the speaker is at a red light at Riverside. It can be interpreted that the rain is in correlation with her sadness and/or tears as to finding herself at a stop in her life, which is in correlation with the red light she is currently waiting on. This assertion can be made by the lines that follow: â€Å"the more I live, the more I think/ two people together is a miracle† (3-4). She is clearly being judgmental about relationships in general based on her experiences in love. Two people together don’t necessarily have to be viewed as a miracle like she puts it. Two women however, in her time, had to have a hidden love, completely unknown to the outside world. The end result would be that their love for each other would have to always stay hidden, as to why she writes: â€Å"The story of our lives becomes our lives† (7). She continues by telling her lover that she knows her response already, and that it most likely will sound like something some Victorian poet would say. â€Å"†¦ salt estranging sea† is what she pulls from Matthew Arnold’s â€Å"To Marguerite† which is mostly stating that we are all alone no matter the millions of people around us. Rich ends the poem by stating that she is in a state of â€Å"estrangement.† Estrangement is separation resulting from hostility, but in this case the estrangement can possibly be synonymous for a fresh start. What can be very  hard to understand about the ending of this poem is that although â€Å"a clef of light† is visible, Adrienne feels more alone than ever. Although the start of something new can be both, good and frightening for someone, th e start of this new day for Adrienne makes her feel colder than she ever has felt. It is interesting to observe that although the speaker is engaging in masturbation, she still feels the coldness that overcame her in poem XVIII.
